Welcome New Patients

Welcome to Elements Holistic Wellness. We want our new patients to feel comfortable and confident in our clinic. As a new patient, you probably have a lot of questions. We plan our initial intake visits to give you time to ask us everything you want to know.

We also make time to get to know you and understand how we can best assist you on your wellness journey. If you have any concerns, please let us know right away so we can make sure to address and accommodate them immediately.   We look forward to meeting you and helping you Live Intentionally in Good Health!

If you would prefer to fill out your paperwork at the clinic, please arrive at clinic 15 minutes early to fill out your paperwork.

What to Bring

We need you to bring just a few things to your first appointment.

  • New patient paperwork

  • Current insurance card

  • Picture ID

  • Cash or Check

Health Insurance

We are preferred providers with many insurance companies. As a courtesy, we are happy to bill your insurance directly. We accept Regence BCBS, Providence Health Plans and MODA.

We no longer accept FSA or HSA payment cards. However, we will provide the correct, coded receipt for you to request your reimbursement.

Were You In A Car Accident?

We bill 3rd party Personal Injury Protection (PIP) section of Motor Vehicle Insurance. No referral needed for acupuncture!

Do I have Acupuncture Coverage?

Contact your insurance provider to learn what kind of care is covered. Here are a few questions to ask:

  • Will my plan cover acupuncture?

  • How many visits per calendar year?

  • Do I need a referral?

  • Do I have a co-pay?

  • Do I have a deductible?

  • If yes, has it been met?

Insurance Billing: As a courtesy to our clients, we are happy to bill your primary insurance for Acupuncture therapy only. We do have pre-paid packages available that offer a nice discount if you do not have insurance. All other services are considered “uncovered” by insurance and will be cash only.
